Health data is everywhere, but it isn’t moving between systems. Patients have to start every new relationship with a provider by telling their health story from scratch.
Founded by a dynamic team of clinicians, patients, and entrepreneurs, Thrive Health’s technology aims to solve the problems of data fragmentation, and reduce patients falling through the cracks of the healthcare system. Their digital healthcare platform puts the patient at the center of their care, allowing patients to build out a dynamic health profile which they can share with providers. The platform also features in-app resources and guides to help patients understand each stage of their care journey, which the hospital and provider can customize according to individual patient needs.

Looking Ahead Care Beyond the Clinic
For 2023 and beyond, Thrive Health is continuing to evolve their technology to support a variety of care environments – not just in hospitals and clinics, but in remote patient settings and workplaces.
Thrive Health’s in-house clinical research team has developed two new custom clinical workflows to support diabetes patients and cancer survivors. These workflows include tailored resources and guides to help patients in each scenario manage their care outside of provider visits, and include specific tasks and trackers so they can upload progress data to their profile as their care evolves. Both diabetes and cancer survivorship are underdeveloped areas in digital healthcare, especially regarding technology that helps patients manage their care remotely. Through partnerships with diabetes and cancer specialists, Thrive Health’s custom-built workflows aim to empower patients undergoing these treatments with the guidance and support they need to live well.
Beyond traditional healthcare environments, Thrive Health is also excited to have launched Circle, a digital care coordination provider that provides full service health and wellness care for employers to offer their employees. Circle is designed to push the boundaries of what traditional corporate wellness offerings can look like, and offers companies on demand, 24/7 access to care providers across both physical and mental health. Circle’s concierge-type services also provides aggregate, de-identified data for employers to track the overall well-being of their workforce over time, and includes added benefits for employees such as prescription delivery, 1:1 counseling, and skincare services.
