Precina helps patients feel better, faster. Our chronic care practice monitors patients’ diabetes every day – just as they do. Our practice combines medical care with social workers to address the complex challenges of controlling diabetes and related comorbidities. We meet with patients consistently and give them unlimited access to our clinicians for any question – all from home. Together with their doctor’s guidance, we make small changes that lead to big improvements in how patients feel – updating patients’ doctors every step of the way. EXPERIENCE IS THE BEST TEACHER
Precina was co-founded by John Oberg, DSW and Dustyn Williams, M.D. based on their joint belief that patient outcomes improve when medical guidance goes beyond prescriptions and dosages to encompass the breadth of challenges faced by patients with chronic diseases.

John started his healthcare career before the age of ten when he was filing patients’ charts in an orthopedic surgeons’ office that his mother managed. In the years since, he’s evolved to a trusted advisor and board member for the largest healthcare leaders and start up innovators in the United States. He founded two Austin-based healthcare innovators, Sedera, a community-based medical cost sharing provider and Precina, a chronic care virtual medical practice currently solving diabetes.

Dustyn entered practice as a hospitalist serving, primarily, patients without health insurance in Louisiana after Hurricane Katrina. In addition to his role as the Chief Medical Officer of Precina, Dustyn is the lead educator at OnlineMedEd, a platform that creates better doctors through better medical education. Frustrated by his clinical experiences in internal medicine treating patients with multiple chronic diseases and the silos of medical education, Dustyn felt passionately that the solution was to address medical, mental health and environmental factors in ways that would encourage patient behavioral change. The old ways of prescribing self-management and medical intervention that patients wouldn’t or couldn’t follow was clearly not the answer.

PATIENTS AND PROVIDERS SHARE GOALS, BUT THE PATH IS WINDING
Chronic disease is complicated to treat. Patients generally want to do the right thing–to get their chronic disease under control. Providers generally understand how to get disease under control in uncomplicated environments. Unfortunately, patients aren’t told how to control disease within the limits of what they are capable of, and providers don’t always know how to solve for non-medical variables. To make matters worse, most patients with complex medical needs don’t have enough time with their providers, and too much time is spent sharing the same information with multiple providers. It is no surprise that many patients with complex chronic conditions do not have them in control. We are starting with diabetes, and soon we will expand to help patients address cardiovascular disease, related mental health issues, and pulmonary disease.

IT’S POSSIBLE. 83% OF PATIENTS CONTROLLED THEIR DIABETES FOR ALMOST 2 YEARS
The Precina methodology was born from a rural Louisiana pilot study led by Dustyn that included fifty people with uncontrolled diabetes. Average patient A1c moved from 9.2 to 6.7 in three months. In approximately 15,000 blood glucose checks, and 15,000 insulin injections, 49 of 50 patients were brought under control and had only one episode of symptomatic hypoglycemia. One patient had refractory diabetes and was referred to an endocrinologist. 83% of pilot study patients controlled their disease for almost 2 years.
